OUR MISSION & VISION

Our mission is to provide comprehensive, goal based, interventional pain management to reduce pain levels and restore functionality. We believe the most effective way to achieve this is by utilizing a multidisciplinary approach, creating an individualized treatment plan for each patient that addresses the physical, functional, and psychological aspects of chronic pain. It is our goal to provide the highest quality of care while enriching the community through education and compassion.

TECHNOLOGY & EXPERIENCE

Our interventional procedure suite allows us to offer on-site treatments using fluoroscopy. Fluoroscopy is a radiologic technique that takes real time images of the body. The image is transmitted to a monitor so that the body part can be seen in detail. Spinal and joint injections are best performed using fluoroscopic guidance to properly target and place the needle (and to help avoid nerve injury or other injury).

About Luis Velazquez, MD

My philosophy is that I am much more than your pain doctor. My goal is that together, you and I push to improve every day to get you back to living the best years of your life.

I completed my undergraduate degree in Biochemistry at University of Miami, and then completed a Masters Degree in Biomedical Science at Barry University. The following year I began medical school at University of South Florida, and once complete, I elected to enter into an Anesthesiology residency at the University of Miami. I thoroughly enjoy the practice of anesthesiology and developed a great deal of pride in my practice. I worked as a trauma anesthesiologist in Miami at a private hospital, and then moved to Las Vegas to practice anesthesiology as well. I am married to an incredible woman, whom I met during my residency in Miami, and she is also a great anesthesiologist. Together we practiced in Las Vegas and found the city to offer a great new opportunity for us; an opportunity for us to completely transform our lives and contribute significantly more to our community during our lifetimes.

This opportunity was the practice of pain management, which I consider one of the most important, yet most poorly understood and mismanaged fields in medicine. I immediately moved to rededicate my life to the pursuit of excellence in treating pain. I accepted an ACGME-accredited interventional pain medicine position at Cedars-Sinai Medical Center in California, and began to develop a deep understanding of the nature of chronic pain, which burdens us not only physically, but emotionally as well.

  • Purpose:

    I am excited to be a part of Velazquez Pain Relief Center in providing comprehensive care using a multi-modal approach to treatment. My goal is to provide patients with the most up to date resources and evidence-based interventions to help them regain functionality. Chronic pain can be debilitating but I aim to deliver the best care possible, so our patients can live their life again.

    Mark A. Chmiela, M.D.

    “Everyone deserves the time and opportunity to be heard and cared for. I strive to provide the right diagnosis so each patient can obtain the right treatment. Together, we will work towards a healthier you.” Dr. Chmiela is double board-certified in Anesthesiology and Pain Medicine by the American Board of Anesthesiology. He obtained his Bachelor’s degree in Biochemistry at the University of Ottawa in Ottawa, Canada, and earned his medical degree from the Jagiellonian University Medical College in Krakow, Poland. Dr. Chmiela completed his residency in Anesthesiology at Cleveland Clinic, and continued as Chief Fellow at Cleveland Clinic to obtain his fellowship in Pain Management. Dr. Chmiela is married and a father of two. His hobbies include fishing and woodworking, and he enjoys camping, watching horror movies, and barbecuing with friends and family.

    Shaan Jaggi, MD

    Dr. Jaggi is double board-certified in Anesthesiology and Pain Medicine. After obtaining his Bachelor of Sciences at Michigan State University he completed his medical school training at Saba University School of Medicine where he received his Medical Degree and Masters of Science. He then completed his Anesthesiology Residency and Pain Management Fellowship at Wayne State University/Detroit Medical Center. Dr. Jaggi believes in multifaceted, patient-centered solutions to managing pain. Every individual struggling with pain is unique, and Dr. Jaggi takes the time to understand the specific challenges and goals for each of his patients. This personalized approach ensures that treatment plans are tailored to address the root causes of pain, providing lasting relief and a path to better overall health. He joined Velazquez Pain Relief Center in 2022.

    Jennifer Tran, M.D.

    Dr. Tran is a board-certified anesthesiologist from Florida. She completed her medical degree and anesthesiology residency at the University of Miami. She joined Velazquez Pain Relief Center in 2015 and provides anesthesia services for our patients.

    Ana Cole, PA-C

    Ana Cole is a board-certified Physician Assistant from El Paso, Texas. She earned her Bachelor of Arts in Sociology from Austin College in 2014, her Masters of Medical Health Sciences from Touro University Nevada in 2021, and her Masters of Physician Assistant studies at Touro University Nevada in 2023. She joined Velazquez Pain Relief Center in 2024. In her free time, Ana enjoys traveling, watching movies, working on art projects, and spending time with her husband and their dogs.

    Leonel Borjon, PA-C

    Leo Borjon is a board certified Physician Assistant from Southern California. He joined Velazquez Pain Relief Center in 2017 following his completion of his Master of Physician Assistant studies at Touro University. Prior to moving to Las Vegas, he graduated from the University of California San Diego with a Bachelor of Science in Human Biology.

    Jason Myers

    Jason Myers is a board-certified Physician Assistant from Las Vegas, Nevada. After serving in the United States Air Force, he received his Bachelor of Science from UNLV. Upon graduating, he pursued his master of Physician Assistant studies at Loma Linda University. Following this, he completed an additional year of training for Orthopedics through Arrowheads Orthopedic Fellowship Program in Southern California. He joined Velazquez Pain Relief Center in 2022.

    Lindsay Manjarres, FNP-BC

    Lindsay Manjarres, FNP-BC, is a family nurse practitioner nationally board certified by the American Nurses Credentialing Center since 2015. Lindsay earned her Bachelor of Science in Microbiology from Northern Arizona University in 2003, Bachelor of Science in Nursing from the University of Arizona in 2005, and her Masters of Science in Nursing from the University of Cincinnati in 2015. Lindsay is a military spouse and has spent many years living abroad. She joined Velazquez Pain Relief Center in 2022.

    Michelle Steinmann FNP-C

    “My philosophy is that you can be the most competent provider. However, without compassion, you cannot make the necessary connection between patients to provide the best possible care.” Michelle Steinmann, FNP-C is a board-certified family nurse practitioner through the American Academy of Nurse Practitioners since 2023. She obtained both her bachelor’s and master’s degrees in nursing from Westminster College in Salt Lake City, Utah. During her nursing career, Michelle worked on the Medical Telemetry Unit at Intermountain Medical Center as a registered nurse and charge nurse from 2020 until 2023. Starting as a new graduate registered nurse during the COVID-19 pandemic was challenging. However, this allowed Michelle to grow and become confident in her skills in treating patients who suffer from chronic pain as well as other medical conditions. She joined Velazquez Pain Relief Center in 2023. She is married to her high school sweetheart, and they moved from Utah to Nevada in 2023. In her free time, Michelle enjoys spending time with her husband, visiting her parents in Florida, and paddle boarding.
