Complete our online form or download and fax our referral pad along with relevant records and imaging to our WC team.

Step 02

Authorization

Our team works with the carrier and adjuster to obtain necessary authorizations prior to the patient’s appointment.

Step 03

Evaluation & Treatment

The injured worker is evaluated and a treatment plan is established. Work status documentation is provided promptly.

Step 04

Ongoing Reporting

We keep adjusters and case managers informed with regular work status updates and medical reports throughout the care episode.

Yes. Our physicians provide clear work status documentation at each visit, indicating whether the patient is at full duty, modified duty, or temporarily total disabled. Return-to-work recommendations are communicated promptly to all relevant parties.
